Vision

No one should face dementia alone. We envision a healthcare system and a society in which every person living with dementia is cared for as a whole person, every caregiver is supported and sustained, and clinicians are united around compassionate, evidence-based care across every culture, age, and setting.

Mission

The Dementia Care Collaborative partners with patients, caregivers, and clinicians to navigate the human experience of dementia together, at every stage of the journey. We advance person‑centered, evidence‑based dementia care by:

Building a culture of compassionate, person-first dementia care

  • We unite clinicians and care teams around shared values, evidence-based practices, and co-management approaches that support whole-person dementia care across settings.

Supporting caregivers through connection and navigation

  • Serving as a trusted resource, we provide personalized and culturally responsive one-on-one guidance, alongside emotional and social support, to help individuals navigate changing needs over time.

Providing evidence‑based education and skills training 

  • We deliver accessible education and skill‑building courses for patients, caregivers, and clinicians–focused on earlier diagnosis, evolving treatments, caregiving roles, communication, care planning, and the realities of day‑to‑day life with dementia.

Advancing learning through research and evaluation

  • We generate and apply evidence to strengthen care models, inform best practices, and continuously improve how dementia care is delivered across the healthcare system.

The Dementia Care Collaborative is part of the MGH Division of Palliative Care & Geriatric Medicine and is an umbrella for the Dementia Caregiver Support Program and the Memory Care Program.
