Collaborators for Change
The Dementia Care Collaborative is a web of hospital-wide communication and education programs to better serve and support our patients with dementia and their caregivers.
With the strong backing of hospital leadership, our team is moving forward to create an integrated, wholistic system to support our fellow colleagues to effectively diagnose, advocate, and care for patients with memory concerns.
The Dementia Care Collaborative has two main programs: The Caregiver Support Program and the Memory Care Initiative. As we transform memory care, MGH is becoming an Age Friendly Heath System and is developing a hospital wide plan to improve dementia care as mandated by the state.

Age Friendly Hospital Initiative
MGH is part of a national Age-Friendly Health System (AFHS) movement! AFHS is new model for patient centered care for older adults. The framework, What Matters, Mentation, Medications, and Mobility is spreading to all US hospitals and medical practices. MGH has achieved Level One recognition as an Age Friendly Hospital, which has led to major changes for our patients and caregivers.Massachusetts Hospital Dementia Legislation
In August 2018, the MA state legislature was the first in the nation to pass critical legislation around one of the most under addressed public health threats. After years of hard work and a solid collaboration among legislators, hospitals, advocates and doctors, H.4116 “An Act relative to Alzheimer’s and related dementias in the Commonwealth” was passed.Caregiver Support Program
Founded in 2017 as a part of MGH’s Palliative Care and Geriatric Medicine Division, the program teaches practical skills to caregivers, provides emotional support, offers connections to community resources and health and resiliency programming.Memory Care Initiative
