Individualized Care Consultation sessions offer an opportunity to discuss each family member’s specific concerns and their family’s needs. The Social Worker will provide education about memory disorders and/or a specific diagnosis if it has been determined. This is essential to understand what the person with dementia is experiencing and what to expect in the future. You will also learn non-medical approaches to challenging situations; strategize for day-to-day problem solving; and identify appropriate resources for immediate and future needs.
